Great Life work Balance position with excellent benefits! We are seeking an MDS RN - Enrollment who is passionate about providing quality care to the geriatric population. Functions as an integral part of an enrollment team to prioritize, organize, and adapt the PACE enrollment process to meet potential participants’ individualized needs and to ensure census goals are achieved. Works as a clinical liaison between prospective participants/caregivers, the PACE enrollment team, the PACE interdisciplinary Team (IDT) and referral sources/other community providers. This is a full-time position, Monday- Friday 8-4 position; no nights, weekends or holidays! 12 paid holidays off! This is not a remote position.
